Habilitation services consist of individualized services and supports that assist with acquisition, retention, or improvement in skills related to living independently in the community. Habilitation service plans are based on current functioning level and are designed to foster the acquisition of daily life skills, social skills, greater independence and personal choice.
Habilitation services are provided in collaboration with case management services within Polk, Dallas, Warren and Madison counties through Broadlawns Medical Center and Eyerly Ball. To access habilitation services or find out if you are eligible, contact Broadlawns at (515) 282-6770 or Eyerly Ball at (515) 241-0982.
